Surrey United Soccer Club - Youth / Mini

Registering as a member of the Surrey United Soccer Club is a simple process! Here are the steps to follow:

Step 1 - Download and print the Registration Form
Download the registration forms from the Forms & Notices page. Please note that there are separate registration forms for mini players (U-6 to U-10), recreational & intermediate players (U-11 to U-18), and select players (U-9 to U-18).
All our forms and notices are in PDF format for which the free Adobe Reader is required. Click on the Get Adobe Reader graphic to download and instal a free copy if you don't already have it.

Step 2 - Fill out and sign the Form
Don't forget to read the Registration Facts. 

Step 3 - Pay the fees, as per the form
Two cheques need to be written for each registration - one for the registration fee, and another post-dated as a uniform deposit. See the registration form for details. Make cheques payable to SUSC.

Step 4 - Mail everything to us!
Players are not considered registered until the registrar received both the form, and payment. You can send everything to:
